Substance Abuse Case Manager
Job Summary: The position is responsible for managing an assigned caseload of adults with substance use disorders. Case Management is tasked with assessing client needs, developing, implementing, and evaluating progress and service plans. Additionally, case management will coordinate and collaborate with SONTX staff and other organizations in meeting client service needs.
DUTIES AND RESPONSIBILITIES: • Maintain accurate documentation of service objectives and outcomes as well as other services in accordance with SONTX and all applicable licenses, regulations, and accreditations. • Maintain collaborative relationships and communication with referring agencies to formulate appropriate strategies that will meet the needs of the client. • Attend program-related community, coalition, and committee meetings as assigned. • Adhere to strict boundaries and professional ethics in the care of others. • Performs other duties as assigned by supervisor, which are consistent with the position and in compliance with SONTX policies and procedures.
CLIENT SUPPORT: • Act as a liaison, advocate, and coordinator for client involvement in 12 step development, programming, and learning independent life skills. • Facilitate open and supportive communication with clients, families, and other organizations and agencies. • Provide ongoing face to face reviews with clients to assess the progress made in reaching goals and revise goals as necessary to ensure completion. • Provide and ensure communication between departments. • Implement and follow-up on all support service directives and/or recommendations.
MEETINGS/FUNCTIONS: • Coordinate and/or attend as needed all client/family/agency related meetings; ensure all necessary follow-up/documentation is completed. REPORTS/DOCUMENTATION/PAPERWORK: • Maintain client related data systems; • Ensure completion and submission of all required daily, bi-weekly. Monthly. Quarterly and annual reports or reviews as scheduled. • Routinely review client charts, ensuring that they are thorough and complete maintaining an organized system.
IMMEDIATE SUPERVISOR(S): Director of Residential Services
EXPERIENCE AND TRAINING: • Extensive knowledge in 12 Step Recovery Programs • 4 year degree in human services or comparable experience of education\work experience required. MACHINES, TOOLS, EQUIPMENT UTILIZED: Must have a Texas State driver’s license, personal vehicle, auto insurance, and cell phone.
JUDGEMENT/INITIATIVE: Must be capable of functioning independently with minimum supervision. Must be able to develop and initiate new programming and use good professional judgement in ongoing decision making.
COMMUNICATION SKILLS: Excellent oral and written skills required for ongoing communication with clients, staff, and community agencies.
